Before you arrive
Arrive 10 minutes early with these ready.
Bring The essentials
So we can set up your file correctly the first time.
Your insurer and plan number, if you'd like us to direct bill your extended health plan.
For ICBC or WorkSafeBC patients — this is all we need to bill them directly.
Something you can move in that gives access to the area we're treating — shorts for knees, a tank for shoulders.
Prepare Worth two minutes of thought
What worries you most? What do you want back — sleep, sport, lifting your kids? Tell us.
When it started, what makes it better or worse, and anything the therapist should know about how it behaves.
Current medications, past surgeries or conditions — they can all shape the safest treatment plan.
X-ray or MRI reports and any referral notes, if you have them — helpful, but not required.
